Admissions

It is the policy of Shrewsbury Nursing and Rehabilitation Center to admit and treat all residents without regard to race, national origin, religion, sex, age, or financial status.

Shrewsbury Nursing and Rehabilitation Center is licensed by the State of Massachusetts Public Health Department as a Nursing Home for Chronic and Convalescent Care – Skilled Nursing Facility. That includes Rehabilitation and Hospice Care in additional to Nursing. All beds are certified for participation in the Medicare and Medicaid programs.

Persons interested in having prospective residents considered for admission to the facility should obtain the “Application for Admission,” the “Authorization for Release of Information,” and the “Masshealth (Medicaid) Questionnaire” forms from the Admissions office. You will find the full application packet that contains all of these forms below or you can call us at 508-845-6786 for more information.

Applicants on the waiting list are offered admittance to Shrewsbury Nursing and Rehabilitation Center in the order that the vacancies occur. An applicant offered admission must be seen by his/her physician within 1 year prior to admission.
